Holistic Support Services proudly partners with Trilogy Care, a registered aged care provider, to deliver high-quality and compassionate services. While Trilogy Care handles the registration and compliance, Holistic Support Services ensures exceptional day-to-day care tailored to your needs. Together, we provide reliable, person-centered support that prioritises your well-being and independence.
Onboarding Process
Complete Online Questionnaire & Sign Home Care Agreement
The questionnaire will include important information to assist the Trilogy Care team review your care needs, including an updated medical and medication health summary.

At the end of the questionnaire, there will be a Home Care Agreement the package holder will need to sign .
Attend Care Plan Meeting
After the Questionnaire and Agreement are submitted, Trilogy Care will send the nominated contact a Care Plan Booking Email. A date & time will need to be chosen for the over-the- phone care plan meeting with your intake assessment officer. During this meeting, a care plan and budget will be created.
Once the steps above are completed, services listed on the care plan and budget can commence on the start date and the clients, or you can commence sending invoices to Trilogy Care for payment.
